بررسی اثر تجویز ترامادول قبل از القای بیهوشی عمومی در پیشگیری از لرز پس از عمل

چکیده زمینه و هدف: لرز پس از بیهوشی عارضه ی شایع پس از عمل است. اثر نسبی مداخلات درمانی در پیشگیری از این عارضه نامعلوم است. هدف از این مطالعه، ارزیابی اثر تجویز ترامادول یک میلی گرم به ازای هر کیلو گرم قبل از القای بیهوشی عمومی در پیشگیری از لرز پس از عمل بود. روش بررسی: مطالعه به صورت کارآزمایی بالینی تصادفی، دوسوکور و دارونما-کنترل، انجام شد. 60 بیمار با وضعیت فیزیکی i یا ii درجه بندی انجمن متخصصین بیهوشی آمریکا که تحت جراحی های شکمی انتخابی زنان با بیهوشی عمومی بودند، مورد بررسی قرارگرفتند. قبل از القای بیهوشی عمومی در 30 بیمار (گروه مطالعه) یک دوز ترامادول وریدی (یک میلی گرم به ازای هر کیلو گرم) و در 30 بیمار دیگر (گروه دارونما) سالین 9/0 درصد تجویز شد. یافته ها: در گروه مطالعه 3 بیمار (10 درصد) و در گروه دارونما 17 بیمار (66/56 درصد)، لرز پس از بیهوشی داشتند (0001/0p<). شیوع درد در مرحله ی بلافاصله بعد از عمل در گروه دارونما به طور معنی دار ی بیشتر بود ( به ترتیب 90 درصد در مقایسه با 33/23 درصد بیماران) (0001/0p<). اختلاف معنی داری از نظر شیوع عوارض جانبی بعد از عمل بین دو گروه نبود. نتیجه گیری: این مطالعه استفاده از ترامادول به میزان یک میلی گرم به ازای هر کیلو گرم قبل از القای بیهوشی را در پیشگیری از لرز پس از بیهوشی حمایت می کند.
